NHS England Foundation Trainee Pharmacist Programme (FTPP) Supervisor Registration Form and Change Form

Registration/Change form for the 2025/26 NHS England Foundation Trainee Pharmacist Programme (FTPP)

The registration / change form enables the following to update NHS England of any personal, programme and/or supervisor changes. It is intended for use by the  

(i)                Foundation Trainee Pharmacists (FTPs or trainees),

(ii)            Supervisors: Designated Supervisors (DS) and Designated Prescribing Practitioners (DPPs) and

(iii)         Educational Programme Directors (EPDs)/Educational Leads (ELs)

The following changes can be made via the form:

Foundation Trainee Pharmacists:

  • Update personal information
  • Change start and end dates if these are wrong in the e-portfolio
  • Change training placement
  • Add, replace and/or remove supervisors
  • Extend training
  • Withdraw from the programme

Supervisors:

  • Register for the first time as a supervisor, for one or more Foundation Trainee Pharmacists
  • Add a new trainee who is starting during the autumn window (20 Oct - 10 Nov)
  • Change role, for one or more Foundation Trainee Pharmacists (eg, change from DPP to DS and DPP)

Educational Programme Directors (EPD)/Educational Leads (EL) who are also supervisors:

  • Register for the first time as a supervisor, for one or more Foundation Trainee Pharmacists and include Educational Programme Director/Educational Lead as a role
  • Add a new trainee who is starting during the autumn window (20 Oct - 10 Nov)
  • Change role, for one or more Foundation Trainee Pharmacists (eg, change from DPP to DS and DPP)

Educational Programme Directors/Educational Leads ONLY:

  • Register as an Educational Programme Director/Educational Lead

The information we are requesting in the forms includes personal identifiable data. Some of the details of this form will be passed onto:

  • the General Pharmaceutical Council, (the pharmacy regulator) [Designated Supervisor only]
  • the local training provider that will be supporting your trainees learning in practice [Designated Supervisor only]
  • our e-portfolio provider for access to the system.
